- The Cosmetic Elite program is designed to provide special incentives and rewards for participating patients.
- We reserve the right to change these terms from time to time, and you agree that each time you use the Cosmetic Elite program card that you accept the terms as they exist at that time.
- Participation in the Cosmetic Elite program is not open to our employees, family of employees, or former employees.
- An annual membership in the Cosmetic Elite Program entails a one-time administration fee of $125. Renewal is available for $100.
- The Cosmetic Elite Program cards are not debit, credit, or charge cards.
- The Cosmetic Elite program stamp card is a reward card only, and only the associated program member can earn benefits upon presentation of the card.
- The Cosmetic Elite program benefits are not valid with any other specials, vouchers, discounts, or in-office offers. Benefits are non-transferrable.
